About the Role:

Responsibilities:
  • Assess client needs and develop personalized care plans to enhance their well-being.
  • Coordinate and oversee the delivery of care services to ensure quality and compliance.
  • Communicate regularly with clients, families, and healthcare providers to ensure continuity of care.
  • Monitor client progress and adjust care plans as necessary to meet evolving needs.
  • Provide training and support to caregivers to ensure high-quality service delivery.
  • Maintain accurate documentation and records of client interactions and care plans.
  • Participate in team meetings and contribute to a collaborative care environment.
  • Stay updated on industry trends and best practices to enhance service offerings.
Requirements:
  • Bachelor's degree in Nursing, Social Work, or a related field preferred.
  • Minimum of 2 years of experience in care management or related healthcare field.
  • Strong understanding of home care regulations and best practices.
  • Excellent communication and interpersonal skills for effective client interactions.
  • Ability to work independently and prioritize tasks in a fast-paced environment.
  • Compassionate demeanor with a commitment to client-centered care.
  • Valid driver's license and reliable transportation required.
  • Current CPR and First Aid certification is a plus.

Compensation: $25.00 - $30.00 per hour
